Confidentiality & Privacy Policy
The law protects the relationship between a client and a psychotherapist, and information cannot be disclosed without written permission.
Exceptions include:
- Suspected child abuse or dependent adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.
- If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
- If a client intends to harm himself or herself, I will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety.
Additional Office Policies and Procedures are as follows:
STANDARD FEE: The fee is $125 per session. Sessions are typically once a week. Frequency will be agreed upon between therapist and client.
PAYMENT: We appreciate your payment at the time of your consultation. Payments may be made in cash, credit card (MasterCard, Visa, Discover, American Express), or check made payable to TFC.
CANCELLATIONS: There are limited numbers of appointment spaces and there is a waiting list. Therefore, a twenty-four (24) hour notice is required for a cancellation, otherwise a fee of a session rate will be charged. This is a standard private practice policy.
MATERIALS: We gladly loan books, articles, and tapes. Please return all borrowed materials within a week , since others are often waiting for their return.
TESTING: Testing and/or other types of evaluation may be necessary. The charges for such testing varies with the test administered and are in addition to the psychotherapy charges.
INSURANCE: We are providers for several insurance companies and Employee Assistance Programs. However, it is the responsibility of the client to verify coverage and obtain authorization information prior to beginning counseling services
